Please download, print, fill out new forms for each event, and return them to Student Ministries.  

• Liability Form: Required for ALL events
• Medical Form: Required for day trips, overnight trips, and multi-day events but not for events at SCC.

HINT: Fill out and save these two PDF documents and then you can quickly update, print and sign for each event!
