Here's "The Deal"!
Liability Form + $ = Registration
(An e-mail or phone call will not register nor hold a spot for your student.)
First come first served. If the deadline arrives and we have available space we reserve the right to extend the deadline.
1. Go to the Special Event Page and get your event information.
2. WRITE a check for the event cost.
3. DOWNLOAD, print, and fill out this Liability Form.
4. SEND both the check and the Liabilty Form to the Student Ministry Office.
"The Catch" is there are a few more things you need to do later:
1.Ya gotta read the Event Information sometime to get the details.
2. Fill out and turn in the other forms by deadlines.
HINT: FILL OUT AND SAVE THESE MICROSOFT WORD DOCUMENTS OF THE LIABILITY AND MEDICAL RELEASE FORMS. PRINT AND SIGN FOR EACH TRIP!
Junior High Ministry (JHM) Event Eligibility
Students must have COMPLETED 7th or 8th grades in order to participate in JHM Summer of 2008 events.
Students who just completed 6th grade and will be in the 7th grade in the fall of 2008 will move up near the end of August.
JHM Spring Service Retreat
Where: Special Olympics, UT, Arlington
When: May 23 through May 25, 2008
Deadline: May 4
Leave and Sign In: 6 p.m. Friday SCC Front Parking Lot
Return: 3 p.m. Sunday SCC Front Parking Lot
Cost: $45 plus meals, t-shirts, etc. . .
To Register Return this Liability form and a check for $45 to the Student Ministry Office.
Then turn these forms in by May 18.

JHM Summer Lock-In
This is our first summer event! Join us for an all nighter of games, videos, food, fun and friends. There will be contests and prizes galore! Make your plans now to be there.
Date: June 6-7
Deadline: June 1
Time: 8 p.m. - 7 a.m. Please eat dinner before you come. Boys bring a salty snack and girls bring a sweet snack.
Where: The Hub
Cost: $25
To Register: Fill out the Liabilty Release and Medication form and turn it in with your $25 check.
JHM Summer Camp
Trip is full and there is a wait list.

First come first served. If someone moves off the list we will start calling down the list until we find someone who can:
1. Actually go on the trip. 2. Write a check for $495.

3. Wait for a phone call. It may come a few days before the trip or the day before if someone else backs out at the last minute.
Where: YMCA of the Rockies, Estes Park, Colorado
When: June 11 - June 18, 2008
This trip is full. Waiting list only at this point.
Who: Students who have completed 7th or 8th grade are eligible to go with JH to the YMCA of the Rockies in Estes Park, Colorado.
Cost: $495 - Payment of a $100 non refundable deposit and a liabilty form is required to hold your child's spot and is deducted from the $495. You may make payments or pay in a lump sum. Balance is due in full by June 1, 2007.
For the sixth year in a row, we will again be making the epic and adventurous trip to stay at the YMCA of the Rockies Camp and Conference Center located in Estes Park, Colorado. The week will be rounded out with Bible-study, opportunities to recreate, and worship around a campfire. All beneath the grandeur that the Lord determined to create in this part of the United States!

JHM Whacky Game Day

WHERE: Warren Athletic Park, Frisco, TX - Parents drop off and pick up at park
COST: $10
TIME: 9 a.m. - 1 p.m.
Whacky? Games? You bet! New Rules! No Rules! Odd Rules! Competition! Come play games, laugh, enjoy time with friends and staff then relax and have some pizza!

JHM Home Team Service Project
WHAT: JH and Home Team combine efforts on service projects for our community.
WHEN: Saturday, July 19
TIME: 8 a.m. - 5 p.m.

A great day full of team work, service, learned life skills, fellowship and just good ole service to others!

JHM Mission Trip 2008

JHM Pool Party
WHEN: Tuesday, August 5
TIME: 6 p.m.-10 p.m.
WHERE: Local Home
WHAT TO BRING: Boys bring 2 liter drink and salty snack, girls bring 2 liter and a sweet snack.
COST: $5
Parents drop and pick up at local home. Maps provided Sunday before in the HUB.
