We reserve space based on the actual number of students paying in advance of the events and turning in their forms.
First come first served. If the deadline arrives and there is available space, we reserve the right to extend the deadline.
1. LIABILITY FORM ONLY for Frisco events or onsight events.
2. LIABILITY FORM AND MEDICAL for events to outlying areas. Forney, Arlington, etc...
3. OUT OF TOWN TRIPS EVENT SPECIFIC FORMS - each of these trips has a different set of forms. Please download them below. DEADLINE Two Weeks Prior to Trip: MEDICATION FORM AND INSURANCE CARD COPY
4. SEND a check, required forms in an ENVELOPE to the Student Ministry office; Attention: Kathy Goodwin. OR place all of the above in an ENVELOPE and drop in the METAL BOX IN THE HUB.

NEW LIABILITY AND MEDICAL FORMS
As of September 1, 2009, we have a new version of the Liability Form and of the Medical forms. We are now requiring a copy of your insurance card with the medcial forms. Please download and USE the new version of the forms for all events.
We are asking for this information in the event of a trip to the emergency room while we are on a trip or overnight event. Thank you!
JHM FUSION
FUSION meets September 9 - December 16, 2009, then January 13 - May 19, 2010! We meet in the HUB, Room 181, from 6:30 - 8:30 p.m.
We take the summers off to allow for summer ministry trips and events.

Come join all your JH friends for worship, small groups, snacks and a great time together! We worship our God, listen to words from the Bible, discuss the lesson in small groups and enjoy some fellowship along the way!
Bring a friend or two and come on in to the HUB (Room 181/82) from 6:30 - 8 p.m. every Wednesday during the school year!

Ladies!!! Join us for an event created just for you! We’ll have special guests, awesome clothes, great music and all the tips and tricks you need to make a statement and a splash without losing your wallet and your heart at the same time. Don’t miss this one of a kind event!
Moms and sisters welcome! There will be fun activities and a cool yet funny skit performed by our high school girls.

JHM SOS MISSION TRIP IS FULL
Invest a week…change a life. Join us as we make our annual JH only missions trip to Service Over Self in Memphis, TN. We'll be rebuilding, remodeling and investing in the lives of people in the Binghamton neighborhood of Memphis. Come be challenged like never before with this opportunity for missions.

JHM SUMMER CAMP TO ESTES PARK, CO
Just as you come over the hill into Estes Park you are blown away by the sight of mountains as far as your eye can see. Now hang out with over 70 of your best friends as we worship, hike, swim, learn and worship for a week like never before. Yep, that's summer camp! Spots are limited so sign up ASAP!

Please turn all medications in by May 15 in the original pharmacy or over the counter bottle. No pill in baggies or containers with no label etc... they must be in the original bottle with dosage and label intact. Thanks.
If there is a change in medications between May 15 and the trip please bring them to us as soon as possible. If this happens the day of the trip please fill out a second medication form and bring it to us by 9 AM Friday morning.
