JHM Summer Camp

What: JHM Summer Camp to Estes Park
When: June 22-29, 2012
Where: Meet in Building A at the Parkwood entrance doors and then we will drive to the YMCA of the Rockies in Estes Park, Colorado.
Time: 6-7 p.m. (Be on time we leave at 7 p.m.)
Cost: $550 You may pay in full or pay the deposit: $110, then four payments of $110.
Deadline to register: June 3rd or as long as spots are available.
DROP OFF REGISTRATION:
You can register by mail, in person, or by using our drop box in the HUB. In order to be registered and hold your space you must send a check with the Liability form to ATTN: Kathy Goodwin

Be sure to type the student name in the first two blanks as the participant. The second page may list the parent name as participant. Please ignore that box. When registration is completed, the student name is listed as participant.

Other Forms Needed by June 18: Download medical form, rules form, and, in case of an emergency, a copy of both sides of your insurance card.
You will need to be at the parent meeting on June 18 to sign a notarized release form.
