Here's "The Deal"!

Liability Form + $ = Registration

(An e-mail or phone call will not register nor hold a spot for your student.)

First come first served. If the deadline arrives and we have available space we reserve the right to extend the deadline.

1. GO to the  Special Event Page and get your event information.

2. WRITE a check for the event cost.

3. DOWNLOAD, print, and fill out this Liability Form.

4. SEND both the check and the Liabilty Form to the Student Ministry Office.

"The Catch" is there are a few more things you need to do later:

1.Ya gotta read the Event Information sometime to get the details.

2. Fill out and turn in the other forms by deadlines.

HINT: FILL OUT AND SAVE THESE MICROSOFT WORD DOCUMENTS OF THE LIABILITY AND MEDICAL RELEASE FORMS. PRINT AND SIGN FOR EACH TRIP!
