Simply Registration
Here's "The Deal"!
Forms + $ = Registration
(Medical not required for events held at Stonebriar)
(An e-mail or phone call will not register nor hold a spot for your student.)
Go to the Special Event Page and get your event information.
Required Forms + Check = Registration Completed
We reserve space based on the actual number of students paying in advance of the events and turning in their forms.
First come first served. If the deadline arrives and there is available space, we reserve the right to extend the deadline.
1. Liability Form Only for Frisco events.
2. Liability Form plus Medical Form for out of town events. Including Forney, Arlington, etc...
3. Out of town trip have event specific forms such as Rules Forms, Liability for the Location - each of these trips has a different set of forms. Please download them below.
4. Deadline Two Weeks Prior to Trip: Medication Form and insurance card copy
5. Send a check, required forms in an enevelope to the Student Ministry office; Attention: Kathy Goodwin. OR place all of the above in an envelope and drop in the metal box in the HUB.
