Stonebriar Community Church requires that all volunteers go through a background check and complete a ministry service application.

Please fill out Online Application which includes a Release Authorization.

Print the Release Authorization for background check (required for all volunteers).  To print the application form for the ministries in which you wish to serve, click on the applicable forms below:

When you have completed the forms, please return them to the church at your earliest convenience using one of these methods:

• To the church office, Monday–Friday, 8:30 a.m.–5 p.m.
• To the ProjectServe kiosk on Sunday mornings

When your forms have been received and you have passed the background check, you will be contacted by the appropriate ministry.
